Job summary

BayCare HomeCare in Largo is seeking an RN Case Manager - Home Health to coordinate patient care with a focus on quality, cost-effectiveness, and strong customer service. The role is PRN with hybrid work options.

The ideal candidate will hold Florida RN licensure, a nursing diploma, and prefer case management experience. This position is part of BayCare HomeCare, a division of BayCare Health System, located in Largo, FL.

Qualifications

• RN license in Florida.
• Associate's or Technical Diploma in Nursing.
• Case management experience preferred.

Responsibilities

• Ensure delivery of quality cost-effective health care through daily utilization review activities.
• Provide individualized case management for patients with complex medical needs.
• Review intake referrals and coding to ensure proper reimbursement from payors.
• Process referrals and orders for authorization.
• Assess clinical appropriateness and safety for home health services.
• Obtain authorizations from MCO payors.
• Promote evidence-based clinical care and clinical excellence.

Case Manager - RN - Home Health Responsibilities include :

• Responsible for ensuring delivery of quality cost effective health care through daily utilization review activities.
• Provides for individualized case management for those patients who have complex medical or catastrophic conditions which have precipitated the need for extensive and possible non-traditional medical support.
• Review of intake referrals, ICD-9 coding, CPT coding to ensure full reimbursement from payor.
• Process referral and additional orders for authorization.
• Review for clinical appropriateness and safety for home care.
• Responsible for obtaining authorization from MCO payor.
• Promotes evidence-based clinical care that exemplifies the pursuit of clinical excellence.

Home Health RN Requirements:

• RN (Registered Nurse) License
• Driver's License State of Florida (within 30 days of hire)
• Required Associate's or Technical Diploma-Nursing
• Preferred Case Manager experience
