Job summary

Sylvan Shores Health & Wellness Center in La Porte, TX is hiring RNs/LVNs for full-time shifts across the day schedule, including double weekends and 6a-2p, 2p-10p, 10p-6a.

You will provide resident care, follow nursing standards, document reports, supervise direct care staff, and participate in meetings; require RN/LVN license, CPR, flexibility, and at least 1 year of clinical experience. The facility offers competitive pay, PTO, and comprehensive benefits.

Qualifications

• Graduate of an accredited school of vocational nursing.
• Unencumbered RN/LVN license.
• Proof of current CPR certification.
• Ability to work flexible schedule.
• Minimum 1 year experience as an RN/LVN in a clinical setting preferred.
• Excellent interpersonal skills.

Responsibilities

• Follow established standards of nursing practices and implement community programs, policies and procedures.
• Obtain report from nurse being relieved and provide report to nurse coming on duty; record sufficient information to implement appropriate follow-up action as necessary.
• Conduct resident rounds daily, reporting problems to supervisor and initiating corrective action
• Identify safety hazards and initiate corrective action immediately
• Identify resident problems and emergency situations and initiate emergency care per physician’s orders in the absence of a physician
• Provide supervision to direct care associates providing direct resident care
• Attend staff meetings, in-service classes and committee meetings as assigned
