The role blends clinical care with relationship-building and outreach, requiring a licensed nurse who can engage communities and drive service excellence.

Qualifications

• Active LVN or RN license is required.
• Experience in homecare, home health, hospice, or assisted living preferred.
• Sales experience in healthcare or service sectors is advantageous.
• Candidate should be warm, compassionate, and patient-focused.
• Strong community engagement and outreach skills desirable.

Responsibilities

• Build and grow referral relationships through service-driven outreach.
• Work onsite at an assisted living partner community and support residents, staff, and families.
• Visit home health patients as needed to coordinate care and build trust.
• Manage and oversee an on-site wellness center.
• Provide health talks and wellness education to seniors and caregivers.
• Help patients navigate care options and connect with resources.
• Collaborate with clinical and operations teams to ensure seamless services.
• Be proactive in meeting patient, family, partner, and community needs.
