Job summary

Leominster Rehabilitation and Nursing Center is seeking an RN/LPN to provide comprehensive nursing care to residents at our facility. Responsibilities include administering medications, assisting physicians, and educating patients and families.

The role requires vigilance in monitoring vital signs and conditions, documenting changes, and maintaining a safe, comfortable environment. A 1-year RN/LPN experience is preferred, with RN/LPN licensure required.

Qualifications

• RN/LPN license required.
• 1 year RN/LPN experience preferred.
• Licensed Practical Nurse may be accepted with appropriate RN credentials.

Responsibilities

• Provides nursing care to patients as assigned.
• Administers and records medications and treatments as assigned.
• Prepares for and assists physicians with diagnostic procedures and treatments.
• Reinforces health care instructions to patients and families.
• Participates with team in planning, implementing and evaluating care.
• Assists in admission, transfer and discharge of patients.
• Observes and reports vital signs and conditions.
• Maintains patient charts with observations.
• Keeps environment safe and comfortable for patients.
• Assists with unit management and may assume charge of unit.

RN/LPN Registered Nurse Licensed Practical Nurse

Duties and Responsibilities:

• Provides nursing care to patients as assigned.

• Administers and records medications and treatments as assigned.

• Prepares for and assists Physicians and others with diagnostic procedures and treatments.

• Reinforce supports and provides health care instructions to patients and their families.

• Participates with other team members in planning, implementing and evaluating nursing care and other patient activities.

• Assists in the admission, transfer, and discharge of patients.

• Observes and reports patients' vital signs, symptoms and general condition to the appropriate individual(s).

• Records on patient charts observation of patient's responses to nursing care rendered.

• Maintains environmental conditions conducive to the comfort and safety of patients.

• Assists with unit management and/or assume charge of unit as assigned.

Experience:

• RN/LPN Registered Nurse Licensed Practical Nurse:1 year (Preferred)

License:
