• Community Health Network, LTD (CHN) is looking for a Licensed Practical Nurse (LPN) OR Registered Nurse (RN) who wants to
• 1) provide care to someone in their home,
• 2) maintain a consistent caseload
• 3) work closely with an Administrative Team that appreciates them.
  

Homecare Nursing requires strong communication but promotes growth and independence. No productivity levels or 45 bed assignments!

• Person-centered care

• Electronic Health Record experience is a plus!

• Relevant education and training based on the individuals care plan

• Certificate in nursing from an accredited nursing school
• Licensed as a LPN/RN as issued by the State of Ohio-Registered on the State Nurse Registry
• BLS CPR
• High school diploma, High school transcripts, GED or equivalent
