Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility. You will provide total nursing care to residents on your unit, participate in care planning, administer medications, and supervise team members to ensure high-quality resident care.

This role requires an active LPN license in Illinois, graduation from an accredited nursing program, strong communication and teamwork skills, and the ability to work with diverse resident populations.

Qualifications

• Graduation from an accredited nursing education program.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift.

Duties and Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as a Licensed Practical Nurse (LPN)
• Participate in developing and implementing individualized care plans alongside the healthcare team
• Administer medications and treatments as prescribed and evaluate effectiveness regularly
• Maintain an environment that promotes residents’ physical, social, and psychological well-being
• Educate residents and families on care plans and medical procedures
• Monitor team member performance and assist with development, evaluation, and corrective action if needed
• Delegate and ensure completion of assigned nursing tasks to team members
• Stay current with facility policies, procedures, and ongoing nursing education
• Recognize and act on significant changes in resident conditions
• Document care, responses, and relevant information per facility guidelines
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions
• Follow all facility safety policies and procedures

Skills and Qualifications

• Graduation from an accredited nursing education program
• Active Licensed Practical Nurse (LPN) license in the state of Illinois
• No prior abuse convictions
• Strong communication and teamwork skills
• Ability to follow policies, maintain safety, and deliver empathetic care
• Must be able to work with diverse resident populations, including elderly and mentally ill individuals
