RiverWoods Exeter is looking for RN/ LPN Per Diem - All Shifts in Exeter, NH.

Overview:

RN/LPN Per Diem

RiverWoods Exeter is currently seeking a Per Diem RN or LPN to provide direct resident care in a team-focused environment where our residents are at the heart of what we do.

Responsibilities:

  1. Perform professional duties according to facility policies, standards of practice, State of New Hampshire Nurse Practice Act, OBRA, and HIPAA.
  2. Collaborate with interdisciplinary team members to develop and update individualized resident care plans. Participate in care plan meetings.
  3. Implement care protocols, including falls prevention, safety, elopement, behavior management, weight management, skin integrity, bowel/bladder management, infection control, and pain management.
  4. Assess and document changes in residents' physical or mental status; report findings to physicians and the care team. Provide complete clinical data for decision-making.
  5. Provide leadership and supervision for direct care staff. Assign daily tasks and evaluate care results.
  6. Establish positive relationships with team members, residents, and families to promote open communication.
  7. Administer and document medications and treatments following guidelines and policies. Manage narcotics inventory securely.
  8. Support and foster a positive culture for staff and residents.

Qualifications:

  • Possess a Nursing Degree or Diploma from an accredited institution.
  • Experience in long-term care and elderly nursing preferred.
  • Hold a valid RN or LPN license in New Hampshire. IV certification preferred. Maintain CEUs annually. CPR certification required.
