• Valid Indiana RN or LPN License.
• 2+ years of skilled nursing experience.
• On-call availability at least one 8-hour period per month.
• Respond to staffing calls within 60 minutes of contact.
• Remain within 30-minute radius of the facility during on-call hours.

Responsibilities

• Provide professional nursing services including resident assessments, vital signs, and medication administration.
• Assist with restorative and rehabilitative care and daily living instruction.
• Conduct regular rounds and communicate observations.
• Document and report unusual or abnormal findings, care plan changes, and follow-up needs.
• Supervise nursing personnel and provide job-specific guidance.
• Communicate effectively with residents, families, and staff.
