Qualifications

• Strong attention to detail and ability to prioritize tasks effectively.

Responsibilities

• Provide direct patient care, including administering medications, performing treatments, and assisting with ADLs.
• Develop and implement individualized care plans in collaboration with our interdisciplinary team.
• Maintain accurate and detailed records of patient care, including progress notes and medication lists.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
• Collaborate with other healthcare professionals to ensure seamless care transitions.

Job Summary:
