Carlin Springs Health & Rehabilitation is seeking an experienced MDS Coordinator (LPN or RN) to join our team. The role focuses on coordinating and completing the RAI/MDS process to ensure accurate, timely assessments that support quality resident care, reimbursement, and regulatory compliance.

The position requires active Virginia LPN or RN licensure and at least 1 year of MDS experience, preferably in a skilled nursing setting.

Qualifications

• Active Virginia LPN or RN license.
• Minimum 1 year of MDS experience.
• Experience in a skilled nursing or long-term care setting preferred.
• Strong knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and PDPM.
• Excellent assessment, documentation, and communication skills.
• Ability to work collaboratively with an interdisciplinary team.

Responsibilities

• Complete accurate and timely MDS assessments in accordance with federal and state regulations.
• Coordinate the Resident Assessment Instrument (RAI) process.
• Ensure compliance with CMS guidelines and reimbursement requirements.
• Collaborate with nursing, therapy, social services, dietary, and other departments to develop comprehensive care plans.
• Monitor assessment schedules and ensure documentation deadlines are met.
• Review medical records for accuracy and completeness.
• Participate in quality assurance and performance improvement initiatives.
• Stay current with regulatory changes affecting MDS and reimbursement.

Pay

LPN: $44.00 - $49.00 per hour

RN: $101,000 - $110,000 annually

Position Summary

The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI)/MDS process to ensure accurate, timely, and compliant assessments.
