Job summary

Elevare Home Care is building a qualified pool of Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) for PRN, full-time, and part-time home-care opportunities throughout Metro Atlanta. Opportunities are evergreen and scheduled based on client needs.

RNs and LPNs provide skilled nursing per client plans, perform assessments, document care, and collaborate with families and care teams. The role requires Georgia licensure, CPR/BLS, background screening, and reliable transportation for

Qualifications

• Active, unrestricted Georgia RN or LPN license.
• Current CPR or BLS certification.
• Current negative TB screening.
• Ability to successfully complete required background screening.
• Reliable transportation, valid driver's license, and automobile insurance for in-home assignments.
• Strong clinical assessment, communication, documentation, and time-management skills.
• Home care, home health, private-duty nursing, geriatrics, pediatrics, or community-based nursing experience is preferred.
• Ability to work independently while following the plan of care, physician orders when applicable, agency policies, and required RN supervision.

Responsibilities

• Providing skilled nursing services authorized by the client's plan of care and within the nurse's license and demonstrated competency.
• Conducting client assessments, reassessments, supervisory visits, and care-plan reviews when assigned and authorized.
• Monitoring client status, documenting care accurately, and promptly reporting significant changes or concerns.
• Coordinating with clients, families, caregivers, providers, and agency leadership.
• Providing caregiver education, case orientation, or competency observation when assigned.
• Completing timely electronic documentation in CareSmartz360.
• Following infection-control, privacy, safety, emergency, and agency policies.
