Qualifications

• LPN/RN will participate in the delivery of patient care in accordance with regulatory requirements and physician orders.
• Nurses oversee the direction of Nursing Assistants to ensure quality services.

Responsibilities

• Be responsible for the care and wellbeing of residents.
• Assess residents' needs and follow care plans.
• Identify and communicate changes in residents’ status.
• Administer medications and other direct care as prescribed.
• Coordinate admissions, transfers, and discharges of residents.

Skills

Excellent verbal and non-verbal communication skills

Skilled nursing facility experience

CPR certification

Education

Current and unrestricted IL LPN/RN License
