Job Title:RN / LPN Continuous Care (Hospice)
Shift(s):12A-8A,8A-4P, 4P-12A (Per Diem)
Location:Our Hospice Nurses care for patients in the most ideal setting - where the patient resides!
Our office is located in Dunmore, PA.

PRN Flex: Must commit to orientation and training.
Work when you can / when you want.

Position Details:
Continuous Care for hospice patients is offered only during periods of “crisis” to maintain a patient in their home. This advanced level of care is necessary when symptoms are not well controlled on a visit basis and the patient requires sustained nursing care.

The ideal candidate:

• The ideal candidate would work to uphold the quality of life for both the dying individual and the caregiver.
• Our Hospice Nurses are involved in helping patientsand their families feel more comfortable about death and providing them with the emotional support they need.
• Additionally, hospice nurses will do their best in assisting family members to manage any practical details that may be involved when caring for a dying loved one.
• Able to demonstrate optimism, enthusiasm and willingness to work constructively with other health care team members and reflect concern for the well-being of patients and employees.

Experience:

• Having experience with hospice patients or in the hospice environment is a plus.
• RN/LPN Candidate would preferably have (1) year general nursing preferred with current hospice, home health, medical, surgical or critical care experience.
• RN/LPN to perform phone triage and prn home visits for our hospice patients during after hours
• Ability to work within an interdisciplinary setting.
• Understanding of Hospice philosophy, principles of death/dying
• Must have PA Nursing License and BLS Certification
