Responsibilities

Assist patients in navigating post-acute care, aiming to positively impact patient outcomes and referral satisfaction. Develop patient-centered transition plans using evidence-based guidelines, protocols, and metrics. Represent the organization in strategic relationships with health systems, hospitals, physicians, and executive stakeholders.

Qualifications

Education and experience, essential

• Graduate of an approved nursing, therapy, or social work school
• Licensed in the current practicing state
• Two years' demonstrated field experience
• Knowledge of federal, state, and local laws governing Medicare-certified home health and hospice
• Basic technology skills, including mobile device operation

Preferred

• RN, physical therapist, or LPN
• Three years of field experience
• Previous home health or healthcare sales experience

Requirements

• Valid state driver’s license
• Dependable transportation and automobile liability insurance
• Ability to drive safely in all weather conditions
