Job summary

Vivo Infusion in Plymouth and Woodbury, MN is seeking a Registered Nurse or Licensed Practical Nurse to join our medical practice team. The RN/LPN will lead patient education, coordinate care, manage pre/post-visit communications, and assist with medication authorizations under physician supervision.

Qualifications

• Graduate of an accredited Nursing Program.
• Current and unrestricted RN or LPN license in Minnesota.
• At least two years of clinical nursing experience.
• Experience at a specialist practice is preferred but not required.

Responsibilities

• Lead patient education, pre- and post-visit phone discussions with patients.
• Coordinate patient care with other healthcare entities and providers.
• Complete healthcare forms such as disability, FMLA, and return-to-work.
• Handle medication refills and pre-authorizations for complex meds.
• Educate patients and families on MS and lifestyle modifications.
• Document patient interactions in EMR per company policy.
