Job summary

Vivo Infusion in Plymouth and Woodbury, MN is seeking a Registered Nurse or Licensed Practical Nurse to join our medical practice team. The RN/LPN will lead patient education, coordinate care, manage pre/post-visit communications, and assist with medication authorizations under physician supervision.

Midwest Immunology & Minnesota Center for Multiple Sclerosis, a Vivo Infusion Company is excited to invite a motivated Registered Nurse or Licensed Practical Nurse to join our growing team in Plymouth & Woodbury, MN supporting our Medical Practice!

The practice RN/LPN will enhance the quality of care for our patients managing Multiple Sclerosis and other immune deficient disease states. The RN/LPN will collaborate closely with physicians, clinical staff, and administrative personnel to ensure that the practice effectively engages with patients. The RN/LPN will be primarily responsible for leading patient education, medication authorization submission, patient phone triage, pre and post visit patient communication, and other tasks that are supportive excellent patient care as directed by practice leadership.

This individual will value patient safety, clinical excellence and operational efficiency. The practice RN/LPN will serve as a mentor and training resource for all non-provider staff serving in patient care roles.

This position will not support our Infusion Suite.

Compensation:

• Pay Range: $41.00 - $48.00/Hour
• Bonus Plan Target: 5% Annually (Based on performance)

Reports to: Clinical Operations Manager

Primary responsibilities:

• Leads practice in performing phone triage for inbound patient inquiries that require clinical guidance. Performs pre- and post-visit phone discussions with patients.
• Performs patient coordination of care with other healthcare entities such as other providers, home care agencies, hospitals, pharmacies and infusion centers for practice patients.
• Completes patient healthcare forms such as disability, FMLA, return-to-work and others.
• Completes medication re-fill and pre-authorization requirements for complex medications as directed by practice providers.
• Manages Botox clinic requirements such as completing medication inventory, monitoring medication supply, scheduling patients and completing pre-authorizations.
• Educate patients and their families about MS, disease management strategies, medication side effects, and lifestyle modifications.
• Support provider staff in collecting comprehensive patient assessments, neurological examinations, and medication reviews.
• Documents and reviews patient interactions in practice electronic medical record system in accordance with Company policies and procedures and all other applicable regulations.
• Provide emotional support and guidance to patients and their families coping with the challenges of MS.
• Coordinate patient care with other healthcare providers, including other specialists, physical therapists, occupational therapists, and others as needed.
• Supports and understands Guiding Principles of the Company; promotes Company Culture.
• Demonstrates knowledge of State and Federal regulations.

Secondary responsibilities:

• Trains and onboards practice staff members as directed by Practice Manager.
• Manages and ensures appropriate documentation of patients’ medical records are maintained in accordance with Company policy and regulatory agency requirements.
• Performs other duties as assigned.

Qualifications:

Education and Experience

• Graduate of an accredited Nursing Program
• Current and unrestricted RN or LPN license in the state of Minnesota.
• At least two years of clinical nursing experience in a dynamic and challenging setting.
• Experience at a specialist practice is preferred but not required.

• Self-motivated with the ability to work independently as needed.
• Must have demonstrated record of and commitment to safety; dedicated to clinical and customer service excellence.
• Must possess outstanding communication and interpersonal skills with ability to communicate professionally and effectively with other staff members, patients and physicians.
• Proficient knowledge of computers, Microsoft Office and software. Willing and ability to learn healthcare related IT platforms that are essential in the day-to-day operation of the practice.
• Able to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
