Job summary

The Health Center at Bloomingdale, a five-star skilled nursing facility in New Jersey, seeks a compassionate LPN or RN for the 7-3 shift. The center offers competitive weekend differential programs and opportunities across all shifts within a warm, resident-centered environment.

Qualifications

• Current licensure as an LPN or RN in New Jersey.
• Experience in skilled nursing or long-term care is preferred.
• Ability to communicate clearly with residents and families.

Responsibilities

• Assist with resident assessment and care plan implementation.
• Communicate effectively with residents, families, staff, physicians, and external providers.
• Monitor changes in resident condition and document interventions.
• Support medication management and ensure secure storage of medications.
• Ensure safe medication dispensing and maintain orderly medication carts.
• Monitor weight, nutrition, skin integrity, mobility, fall and pain management.

Responsibilities:

• Assist with resident assessment and implementation of care needs and will participate in resident and family care conference

• Must be able to effectively communicate with residents, families, staff, physicians, outside providers and the public

• Ensures that changes in resident condition are evaluated, monitored, documented and effective interventions are implemented

• Provide support and assistance to medication management systems

• Ensure that medications are dispensed in a safe and effective manner, keep medication cart orderly, and all medications appropriately packaged and stored

• Monitor resident health concerns including weight and nutrition, skin integrity, mobility, fall management and pain management

Requirements:

• Current licensure as a Practical Nurse (LPN) or Registered Nurse (RN) in the State of New Jersey
