Job summary

Signature HealthCARE of Parkwood in Carmel, Indiana is seeking a dedicated RN to join our care team. You will provide direct nursing care, manage medications, and support residents withADLs in a skilled nursing setting.

The ideal candidate will hold an active RN license in Indiana, with 1+ year of clinical nursing experience, and preferably some supervisory responsibilities. EHR proficiency and strong documentation skills are important.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during shifts per policy.
• Monitor residents’ status and notify physicians of changes in condition.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy and procedure.
• Coordinate resident admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities and regulatory surveys.
