Healthy Living at Home, a Mission Healthcare Company, is seeking an experienced Registered Nurse to serve as Assistant Director of Patient Care Services in Modesto, CA. You will help implement and lead clinical home health services, directing goals and ensuring quality standards across the organization.

The role requires strong supervisory skills, caseload oversight, and collaboration with clinical directors, physicians, and staff to improve patient outcomes and align with regulatory

Qualifications

• Current unencumbered RN license in California.
• Nursing degree from an NLN-accredited program.
• 3+ years of RN experience; 2+ years in home health.
• 1+ year of supervisory/management experience in home health.
• Current CPR card; ability to supervise professional staff.
• Knowledge of clinical decision-making and quality management.
• Valid driver’s license with insured vehicle.

Responsabilités

• Receives case referrals and assigns appropriate personnel.
• Reviews and guides care plans to ensure quality home health services.
• Attends case conferences to coordinate care with the team.
• Conducts quarterly record reviews and communicates findings.
• Supports interviewing and onboarding of new employees.
• Contributes to policy development and staff education.
• Promotes customer service orientation and community relations.
