Job description

Overview

The Physician Medicine and Rehab Office Nurse Lead completes assigned duties, serves as a vital communication link between the physician(s) and the patients, families, referral sources, physicians' offices, and others and assists the physician(s) as needed at a higher level to support specific specialty services and competencies. Responsible for implementation of programs within the PMR clinic as directed by Clinic Administration, Director, or Physician ensuring clinic policy and procedure manual are current. Mentors/Orients other clinicians as assigned and is responsible for the assessment, planning, implementation, and evaluation of nursing patient care within an outpatient office setting. Will serve on planning committees that involve services offered/or new services to be developed in this PMR clinic when deemed appropriate. The patients to be served include adolescent, adult, and geriatric population with occasional infant or pediatric patient populations. In addition, this associate is expected to adhere to and exemplify the St. Luke's Standards of Excellence and the PMR associate-patient relationship policy.

Responsibilities

Key Accountability-Clinical excellence-patient care

  • Assist provider and other clinical staff with obtaining basic vital information preparing patient for examination (assist with undressing/dressing), treatment, and clinic procedures.
  • Assists provider in patient examinations and treatments/procedures with EMG guidance.
  • Demonstrates competence in the clinical skills necessary to carry out assigned job duties.
  • Apply nursing process (assessment, diagnosis, planning, intervention, and evaluation within licensure guidelines).
  • Administers medications and nursing treatments within scope of practice.
  • Prepares Botox injections per physician orders.
  • Place urinary catheters per protocol and license.
  • Preforms bladders scans as directed by provider.
  • Responsible for tracking system for follow-up appointments related to office visits, laboratory tests and procedures to regularly follow patients monitoring disease status.
  • Provide appropriate and timely documentation in the patient's electronic health record using standardized workflows and processes.
  • Provides support for patient transfers from wheelchair to exam table and back using proper techniques/transfer equipment.
  • Facilitates Baclofen pump refills and/or assist provider with pump refills. (RN only)
  • Understand office procedures and anticipates/coordinates supplies needed prior to and during the exam/procedure.
  • Communicates effectively with discharge planners, insurance carriers, physicians, patient, family members and other related parties.
  • Implements measures to promote a safe environment for the patient/family.
  • Successfully completes the orientation and annual hospital and unit specific competencies.
  • Incorporates St. Luke's Comfort/C.A.R.E. Professional Practice Model in clinic nursing care.

Key Accountability-Care Coordination

  • Responsible for the co-management of patient in a specialize treatment program.
  • Understands and provides education around neurogenic bladder/catheters, neurogenic bowel and other specialized treatments directed by physician.
  • Utilizes the AVS to communicate changes in medication and plan of care for the clinic patient and assures all clinic patients are given a copy of the AVS.
  • Verifies consent forms and patient pain contracts are completed and scanned to electronic chart per protocols.
  • Schedules test ordered by the physician and obtains prior authorizations for testing and/or medications per insurance company policy.
  • Telephone triage and communication of test results to provider, providing and documenting advice for care of illness/conditions per provider direction; conducting timely follow-up phone calls regarding patient's health and progress as necessary. Educates patients in their treatment plan. Assess and chooses teaching strategies appropriate to the patient's and their families' age specific needs, culture, gender, readiness to learn and level of comprehension.
  • Demonstrates knowledge that patient and workplace safety is a priority by preventing errors, accidents and infections and reporting potential or actual concerns immediately.

Key Accountability-Leadership and Professional Development

  • Works with leadership to develop, update and complete clinical nurse competency checklist annually.
  • Assists in the orientation of new associates by acting as a resource and mentor to associates. Collaborates with leadership, as appropriate.
  • Continues professional development and involvement in professional activities that enhance nursing practice by sharing knowledge through networking and research.
  • Utilizes evidenced based models of learning.
  • Participates in department and hospital shared governance infrastructure and performance improvement activities.
Qualifications
  • Graduate of accredited nursing program for RN or LPN
  • Possess a current Iowa RN or LPN license.
  • Previous rehabilitation nursing experience preferred.
  • Writes, reads, comprehends and speaks fluent English

  • Demonstrates proficiency in the manual skills and techniques required to perform basic/advanced nursing procedures in the administration of medication therapy and the operation of specialized equipment
  • Ability to complete a basic computer course on the Electronic Medical Record (EMR) through in-hospital training
  • Use of usual and customary equipment used to perform essential functions of the position.
