Qualifications

• Basic Life Support Certification (BLS) for healthcare providers.

Responsibilities

• Assess physical, social, and psychological status and collect information from the interdisciplinary team.
• Develop and implement an individualized plan of care with patient and family involvement.
• Coordinate care and delegate tasks to the care team to meet the plan of care.
• Evaluate patient response and revise the plan of care as needed, documenting outcomes.
