Job summary

Lifekind Health is seeking an experienced Registered Nurse Lead (RN) to oversee clinical operations and care management at our Victorville clinic. The role blends hands-on patient care with clinical leadership, staff supervision, and ECM oversight.

You will supervise LVNs, CHWs, and Care Coordinators, participate in patient care and care planning, and drive population health initiatives within a transdisciplinary team focused on the quadruple aim.

Qualifications

• Active and unrestricted Registered Nurse (RN) license in California.
• Associate’s Degree in Nursing required; BSN strongly preferred.
• Minimum of 3 years nursing experience, with at least 1 year in leadership or care management.
• Valid BLS or CPR certification.
• Experience with case management or Enhanced Care Management (ECM) preferred.
• Experience supervising LVNs, CHWs, and Care Coordinators preferred.

Responsibilities

• Provide day-to-day clinical leadership and supervision for LVNs, CHWs, and Care Coordinators.
• Serve as clinical point of contact for staff questions and escalations.
• Collaborate with providers and clinic leadership to ensure high-quality care.
• Support clinic workflow optimization and patient flow.
