Job summary

Oregon Health & Science University seeks an RN Transplant Coordinator to provide comprehensive care coordination for transplant patients through evaluation, transplantation, and post-transplant follow-up.

You will work with physicians, social workers, nutritionists, pharmacists, and other team members to ensure timely, patient-centered care and optimal outcomes. Strong communication and organization are essential for this role.

Qualifications

• 2 years of independent RN practice.
• BSN prerequisite and program accreditation 30 days before start date.
• ADN graduates must enroll in BSN within 3 years and complete within 5 years.
• Current, unencumbered Oregon State RN License; WA license within 2 months.
• BLS from AHA; orientation during onboarding.
• Must be able to perform essential functions with or without accommodation.

Responsibilities

• Provide comprehensive care coordination for transplant patients across all phases.
• Assess readiness, education, and coordinate diagnostic testing and treatments.
• Monitor progress and be primary contact for patients, families, and team.
• Collaborate with physicians, AP providers, social workers, nutritionists, pharmacists, and coordinators.

Clinical Transplant Services provides specialized care for transplant patients throughout all phases of the transplant journey, including evaluation, pre-transplant preparation, transplant hospitalization, and post-transplant follow-up. The department consists of four transplant programs: Kidney/Pancreas, Liver, Heart, and Pediatric Kidney.

Department Overview

Clinical Transplant Services provides specialized care for transplant patients throughout all phases of the transplant journey, including evaluation, pre-transplant preparation, transplant hospitalization, and post-transplant follow-up. The department consists of four transplant programs: Kidney/Pancreas, Liver, Heart, and Pediatric Kidney. Each program works closely with multidisciplinary teams to coordinate care. Our goal is to deliver high-quality, patient-centered care while promoting positive outcomes and the efficient use of healthcare resources.

Function/Duties of Position

RN Transplant Coordinators provide comprehensive care coordination and clinical support for transplant patients throughout all phases of the transplant journey. They assess patient readiness, provide education, coordinate diagnostic testing and treatments, monitor clinical progress, and serve as a primary point of contact for patients, families, and the multidisciplinary transplant team.

RN Transplant Coordinators collaborate closely with physicians, advanced practice providers, social workers, nutritionists, pharmacists, financial coordinators, community providers, and other healthcare professionals to ensure timely, patient-centered care. They facilitate care planning, coordinate transitions across care settings, and help address clinical and psychosocial needs that may affect transplant outcomes.

RN Transplant Coordinators support quality patient outcomes and process improvement initiatives by participating in multidisciplinary team meetings and collaborative transplant care activities and are an integral part of the transplant care team.

Communication methods include telephone, voicemail, Microsoft Teams, Epic, and email.

Required Qualifications

• 2 years of independent RN practice
• BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date.
• ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date.
• Associate degree Nurses required to enroll in BSN program within 3 years of hire and complete within 5 years of hire
• Current, unencumbered Oregon State Registered Nurse License.
• Will need to obtain WA license within 2 months of hire date.
• BLS from AHA required. New hires will be enrolled and required to complete during orientation.
• Must be able to perform the essential functions of the position with or without accommodation

Preferred Qualifications

• 2 years of ambulatory experience
• BSN
• Transplant Coordinator or Related Specialty Experience
• Transplant Certification (CCTN or CCTC)
• 3 years of acute-care hospital experience

Additional Details

• External candidates: OHSU & Nursing New Employee Orientation (NEO) for about a week.
• Either Transition to Practice (TTP) Program Specialty Fellowship/Fellowship Learning Pathway or Orientation experience for 3 days-26 weeks depending on the care area. An employment service agreement may apply.
