Job summary

Kaiser Permanente in Clackamas, OR is seeking an RN for IV Therapy / Vascular Access. You will perform multi-system assessments, plan personalized nursing care, and coordinate discharge planning with diverse teams.

This role requires an ADN, an active RN license in Oregon, and experience in hospital settings. You will document in the EMR, participate in quality activities, and collaborate across departments to ensure optimal patient outcomes.

Qualifications

• ADN required.
• Experienced level nursing experience required.
• Current RN License in Oregon.

Responsibilities

• Perform accurate multi-system assessments, including biophysical, psychosocial, and environmental needs.
• Identify deviations from normal on routine lab diagnostic tests and reassess patients throughout hospitalization.
• Develop an individual nursing plan of care sensitive to the developmental needs of the patient.
• Coordinate patient care activities and discharge planning with other departments and community agencies.
• Initiate appropriate measures based on protocol for managing medical and/or surgical patients.
• Correctly carry out the physician's plan of care and accurately document in the EMR.
• Utilize a multidisciplinary approach when appropriate and perform all necessary procedures correctly.
• Evaluate assessment methods, plan of care, and patients' responses to treatment.
• Participate in unit quality assurance activities.
