Mercy is seeking a Care Manager to coordinate discharge planning and care transitions for patients in an acute care setting in Fort Smith, AR. You will work closely with physicians, nurses, and other healthcare professionals to ensure safe discharges and seamless transitions. Required BSN and current RN license.

Working closely with physicians, nurses, and other healthcare professionals, you'll coordinate care, support safe discharges, and help patients navigate their healthcare journey from admission through discharge.

What You'll Do

Schedule

• Preferred schedule: 8-hour shifts on Friday, Saturday, and Sunday.
• May consider 12-hour shifts on Saturday and Sunday based on staffing needs.

Qualifications

Education

• BSN required

Licensure

• Current RN license required.

Experience

• 2-3 years of acute care hospital experience preferred.
• Care Management or Utilization Management experience preferred.

Certifications

• BLS required at hire or within 90 days.
• Case Management certification preferred.

Skills

• Strong communication and critical thinking skills.
• Effective time management and assessment abilities.
• Ability to work collaboratively across all levels of the organization.
• Knowledge of InterQual or Milliman Care Guidelines preferred.
• Ability to provide care coordination across diverse patient populations.
