Job summary

LiveWell Homecare Agency is seeking a qualified Home Infusion Nurse to deliver infusion therapy across clinical and home settings. You will coordinate care, verify patient information, and provide direct patient education while managing casework and collaborating with healthcare teams.

The role requires performing infusion administrations, developing care plans, and supporting on-call coverage with travel to patient homes and clinical sites.

Qualifications

• Minimum of three years RN experience, with home health and IV therapy preferred.
• Graduation from a specialty nursing training program is required.
• Strong clinical assessment, communication, documentation, and teamwork skills.

Responsibilities

• Develop personalized care plans and administer infusion medications.
• Prepare and administer infusion therapies and monitor progress.
• Educate patients and caregivers; coordinate with healthcare teams.
• Maintain documentation; participate in protocol development.

Position Overview

The Home Infusion Nurse coordinates and delivers infusion therapy in clinical and home settings, managing referrals, verifying patient information, and providing direct care, education, and case management.

Key Responsibilities

Develop personalized care plans, prepare and administer infusion medications, perform vascular access procedures, educate patients and caregivers, monitor therapy progress, and ensure effective communication with healthcare teams. Maintain accurate documentation, respond to patient concerns, participate in protocol development, and support 24/7 on-call coverage. Travel to patient homes and clinical sites, oversee licensed practical nurses, and contribute to quality improvement initiatives.

Qualifications & Skills

Minimum of three years RN experience, preferred home health and IV therapy expertise, and graduation from a specialty nursing training program. Strong clinical assessment, communication, documentation, and teamwork skills are essential.
