Position Summary:

Oversees and coordinates care within a specific specialty area for an assigned caseload. Facilitates appropriate care, assists patients and families in navigating and negotiating care options. Ensures optimal utilization of resources and efficient patient progression through the healthcare continuum, aiming for high-quality, cost-effective care.

What you will do:

• Conduct comprehensive health assessments covering physical, psychological, social, environmental, financial, and functional aspects, including past and present treatments. Develop goals, treatment plans, and provider options.
• Collaborate with clinical teams and medical staff on performance improvement activities, including assessment, planning, implementation, and evaluation of clinical outcomes for specific populations.
• Serve as a clinical expert for the populations served, providing education and internal consultation to staff.
• Apply critical thinking and problem-solving skills to analyze data and develop appropriate case management plans addressing individual and family needs.
• Monitor and adjust care plans collaboratively with the healthcare team to ensure optimal patient outcomes.

What you will need:

• Graduation from an accredited Nursing program
• Associates Degree in Nursing required; BSN preferred. If in Grand Rapids and holding an ADN, enrollment in a BSN program is required within 6 months of hire, with completion within 4 years.
• Licensed in the State of Michigan
• Chemo/Biotherapy Certification required
• Minimum of 1 year of RN experience; new graduates must apply to the RN residency program held three times a year (February, July, October).
