Horizon Blue Cross Blue Shield of New Jersey is seeking an experienced RN to ensure appropriate levels of care and coordinate transitions in the continuum of care. You will assess clinical needs, evaluate services, and collaborate with patients, families, physicians, and hospitals to optimize outcomes.

The role offers a Monday–Friday schedule with 8- or 10-hour days in Hopewell, NJ, with a pay range of $79,100–$105,945 annually and comprehensive health benefits.

Qualifications

• RN licensure in NJ or Compact license.
• Minimum 2 years clinical experience.
• Minimum 2 years’ payer experience.

About This Role

This position is responsible for performing RN duties using established guidelines to ensure appropriate levels of care and planning for transitions in the continuum of care. It is ideal for experienced nurses looking to make a significant impact in patient management and care coordination.

Highlights

• Location: Hopewell, NJ
• Schedule: 8- or 10-hour workday, Monday through Friday, varying between 8am and 11pm

What You'll Do

• Assess patient's clinical needs against established guidelines to ensure appropriate care levels.
• Evaluate the necessity, appropriateness, and efficiency of medical services and procedures.
• Coordinate and assist in the implementation of care plans for members.
• Monitor and coordinate services rendered outside of the network and negotiate fees as appropriate.
• Coordinate with patients, families, physicians, and hospitals regarding care appropriateness.
• Advocate for members and families among various sites to coordinate resource utilization.
• Document accurately and comprehensively based on standards of practice and policies.
• Interact and communicate with multidisciplinary teams to ensure continuity of care.
• Evaluate care by problem-solving and participating in quality improvement programs.
• Serve as a mentor/trainer to new RNs and other staff as needed.

Requirements

Education:

• High School Diploma/GED required
• Bachelor degree preferred or relevant experience in lieu of degree

Experience:

• Minimum of two (2) years clinical experience
• Minimum of two (2) years’ experience with health care payer experience

Licenses & Certifications:

• Active Unrestricted NJ RN License or active Compact License required
• CCM certification preferred
