Job summary

Beacon Hospice, Amedisys is seeking an RN Hospice Registered Nurse for PRN Per Diem assignments across the Waterville to Bingham service area. You will perform initial assessments, develop and revise plans of care, and coordinate with the care team to maintain continuity of patient care at home.

The role offers competitive pay of $40.00–$50.00 per hour, with benefits such as PPO or HSA medical plans, PTO, wellness rewards, counseling support, a 401(k) match, and mileage reimbursement.

Qualifications

• Current RN license, state(s) you are assigned to work.
• One year of nursing experience preferred; new graduate option available for RN.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes and revises the plan of care; maintains continuity of patient care with the team.
• Communicates patient progress to the clinical manager, physician, and care team.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes.
• Educates families on patient condition and needs.
• Makes referrals to other hospice care disciplines as needed.
• Performs on-call duties as assigned.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One year of nursing experience preferred. If less than one year of nursing experience, you can be considered for the RN new graduate program.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
