Job summary

Sanford Health in Canby, MN, is seeking a Part-time Registered Nurse for its hospice home health program. You will provide professional nursing care, using the nursing process to assess, plan, implement, and evaluate patient needs in a home setting.

Collaborate with patients, families and physicians to coordinate care, educate patients, and ensure adherence to nursing standards and hospice guidelines. A flexible schedule supports work-life balance in a rural health system.

Qualifications

• Graduate from a nationally accredited nursing program.
• Minimum of one year of professional nursing experience preferred.
• Licensed RN in the state where practice occurs or hold multistate licensure if in an NLC state.
• BLS required.
• CHPN certification preferred.

Responsibilities

• Provide professional nursing services to patients/residents in the hospice home health setting.
• Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation).
• Collaborate with patient/resident, family, and physicians to plan, implement and evaluate care.
• Coordinate and ensure continuity of care, patient education, triage and other nursing interventions as appropriate.

Job Summary

The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide professional nursing services to patients/residents within the hospice setting. The nursing process includes the documentation of human responses to actual or potential health problems in the home health environment. This practice includes dependent, interdependent and independent functions. Collaborates patient/resident and family, other inter-professional colleagues, such as physicians, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned setting. Cares for patients/residents in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up for patients/residents in the hospice setting. Responsible to implement the nursing process, the coordination and continuum of care, patient/resident assessment, patient/resident education, triage, and various other nursing interventions. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

Qualifications

• Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
• Bachelor's degree in nursing preferred.
• Minimum of one year of professional nursing experience preferred.
• Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
• Obtains and subsequently maintains required department specific competencies and certifications.
• Depending on location, a valid driver's license may be required.
• Basic Life Support (BLS) required.
• Certificate for Hospice and Palliative Nurse (CHPN) preferred.
