Compassionate Care Hospice (CCH), an Amedisys Company, is seeking an RN On-call Nurse based in Lakeland, FL with daily travel. Schedule: Monday - Thursday 5PM–8AM and Friday - Monday 5PM–8AM.

You will perform initial nursing assessments and coordinate the care plan, ensuring continuity of patient care with the care team. Qualifications

• Current RN license in the state(s) where you will work.
• Minimum 1 year of clinical RN experience in an acute healthcare setting.
• Current CPR certification.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the total plan of care with continuity of patient care.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Develops and maintains individualized patient care progress records.
• Records pain/symptom management changes/outcomes.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed.
• Performs on-call responsibilities and on-call services to patients/families.

Salary Range

$83,000–$87,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

Required:

• Current RN license in the state(s) where you will work
• Minimum 1 year of clinical RN experience in an acute healthcare setting
• Current CPR certification

Preferred

• Hospice or home health experience
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficient in electronic medical record (EMR) systems
