Job summary

Beacon Hospice, Amedisys is seeking an RN Case Manager to coordinate Home Health and Hospice patient care. You will assess patients, develop care plans with the team, and ensure timely documentation of progress and outcomes.

You’ll educate families and work closely with the clinical manager, with on-call duties as assigned. Join one of the largest home health and hospice providers in the U.S., offering competitive pay and comprehensive benefits.

Qualifications

• One year of nursing experience required.
• Current RN license for assigned state(s).
• Current CPR certification required.
• Valid driver's license and liability insurance.

Responsibilities

• Performs patient assessments and develops a care plan with the team.
• Makes referrals to other hospice disciplines when needed.
• Maintains and documents patient care progress records.
• Records pain/symptom changes and outcomes.
• Educates patients, families, and caregivers in care techniques.
• Communicates with the clinical manager and care team.
• On-call responsibilities and related duties.

• $90,000.00 - $98,000.00 salary

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.


• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.


• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.


• Records pain/symptom management changes/outcomes as appropriate.


• Teaches patients, families, and caregivers appropriate patient care techniques.


• Regularly communicates with clinical manager and care team.


• Performs on-call responsibilities and on-call services to patients/families as assigned.


• Other duties as assigned.

Qualifications

• One year of nursing experience.


• Current RN license, specific to the state(s) you are assigned to work.


• Current CPR certification.


• Valid driver's license, reliable transportation and liability insurance.
