Job summary

AMEDISYS HOME HEALTH is seeking a Hospice RN Case Manager to assess patients, coordinate with the care team, and implement individualized care plans in home hospice settings.

The role offers a competitive salary of $84,000–$87,000 per year plus mileage, and a comprehensive benefits package including PTO, medical options, and a 401(k) with company match. On-call duties and ongoing professional growth opportunities are provided.

Qualifications

• One year of nursing experience.
• Current RN license valid for the state(s) you work.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and accordance with care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
