Beacon Hospice, a part of Amedisys, is seeking an experienced RN to join our weekend team providing compassionate hospice care at patients' homes in East Hartford, CT.

This part-time weekend role focuses on nursing assessments, care planning, and collaboration with the care team to ensure comfort and dignity for patients at home. You will be on-call as needed and contribute to high-quality hospice services.

Qualifications

• One year of nursing experience.
• Current RN license for the state you work in.
• Current CPR certification and valid driver's license.

Responsabilités

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Qualifications
