Job summary

Haven Hospice in Gainesville, FL is seeking a Clinical Community Liaison to build relationships with hospitals, nursing homes, and community providers. You will promote hospice care, identify eligible patients, and coordinate care with our admission team.

This role helps families through transitions with compassion and professional guidance. The position requires RN licensure, clinical nursing experience, and strong compliance knowledge to ensure quality patient care and accurate documentation.

Qualifications

• Registered Nurse with current licensure to practice in assigned state(s).
• Three years’ experience in clinical nursing; hospice experience preferred.
• Two years of experience in compliance, internal audit, basic law or paralegal preferred.
• Excellent understanding of Medicare Conditions of Participation and medical documentation standards.
• Ability to manage highly confidential information; meticulous attention to detail.

Responsibilities

• Meet with hospitals, nursing homes, and community providers to promote hospice care.
• Identify patients who may benefit from hospice services and assess their eligibility.
• Coordinate care between referral sources and our hospice admission team.
• Support patients and families during the transition to hospice care.
• Track local healthcare needs, referral trends, and competitors.
• Participate in outreach events and support marketing with planning and strategy.
