Qualifications

  • 1-2 years of hospice, oncology, or related experience.
  • Experience caring for the terminally ill; knowledge of hospice care.
  • Hospice certification required within 18 months of employment.

Responsibilities

  • Perform referral evaluations and admission assessments for hospice services.
  • Conduct physical, psychosocial, and spiritual assessments.
  • Provide Case Management as part of the Interdisciplinary Group for assigned caseloads.

We Are Looking for Experienced Hospice Nurses
  • Perform referral evaluations and admission assessments for hospice services.
  • Conduct physical, psychosocial, and spiritual assessments, utilizing non-verbal cues and probing questions for comprehensive understanding.
  • Communicate patient and family expectations, goals, and educational needs to the healthcare team, ensuring clear and accurate documentation.
  • Provide Case Management as part of the Interdisciplinary Group (IDG) for assigned caseloads.
  • Deliver services aligned with professional standards, approaching patients and families with care, concern, and compassion.

Minimum Qualifications

Details

Part-Time position, 7 shifts per pay period, 1st shift (7am-3:30pm), including weekend and holiday rotations. Hourly, non-exempt role at the Hospice Inpatient Unit - Lawlis Family Hospice.
