Job summary

Grace Healthcare Services is seeking a Registered Nurse (RN) Hospice Liaison Float to work across RWJ Livingston Campus and Jersey City Campus. The role blends clinical nursing with business development, focusing on building referral networks and coordinating patient care in a contracted hospital setting.

Responsibilities include identifying patient needs, creating comprehensive care plans, and supporting families through transitions.

Qualifications

• Registered Nurse (RN) with a valid license.
• Experience in Hospice or Palliative Care is a plus.
• Strong Communication & Relationship-Building Skills.
• Compassionate and Patient-Centered Approach.

• Build and nurture strategic partnerships with new and existing referral sources to facilitate appropriate referrals.
• Identify and assess the psychosocial, spiritual, and physical needs of patients in the inpatient setting.
• Coordinate comprehensive care plans, ensuring seamless transitions to the appropriate level of care.
• Conduct pre-discharge hospital visits, offering support and guidance to patients and their families.
