Job summary

Morning Star Hospice and Palliative Care in Harrisburg, PA is seeking a dedicated Registered Nurse (RN) Hospice Case Manager to join our compassionate team. You will coordinate home-based and facility-based patient care with an interdisciplinary team to develop individualized plans.

Requirements: active PA RN license and at least 1 year in hospice or palliative care. You will provide skilled nursing, educate families, monitor progress, and advocate for comfort and dignity at end of life.

Qualifications

• Current and unrestricted RN license in Pennsylvania.
• At least 1 year of hospice or palliative care experience preferred.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care including medication management, pain control, symptom management, and emotional support.
• Act as a liaison between patients, families, and the interdisciplinary care team to ensure coordinated care.
• Educate patients and families on disease processes, medications, and end-of-life care options.
• Monitor patient progress and adjust care plans as needed to ensure comfort and dignity.
• Document patient care and progress in compliance with hospice regulations and company policies.
• Participate in regular team meetings to discuss patient status and plan of care.
