Ethos Hospice is seeking a dedicated RN Hospice Case Manager in Dallas, TX for a full-time, permanent position with comprehensive benefits and a competitive bonus structure. You will coordinate all aspects of patient care, develop individualized plans with the interdisciplinary team, and perform thorough assessments while managing medications.

The ideal candidate holds an active RN license, CPR certification, a valid driver's license, and has hospice experience.

Qualifications

• Current Registered Nurse (RN) license is required.
• Two years of RN experience in a clinical care setting.
• One year of full-time hospice experience preferred.
• Current driver's license is required.
• Current CPR certification is required.

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs, and care.
• Communicate patient progress with clinical leadership, physicians, and the care team.
