Job summary

Amedisys Home Health is seeking an RN Hospice Case Manager to deliver patient-centered care in the home setting. You'll perform assessments, develop care plans with the hospice team, and coordinate referrals to other disciplines to ensure comprehensive support.

This role offers a competitive salary ($84,000 - $86,000) plus mileage, with benefits and opportunities for growth. Territory coverage includes Winston-Guilford and Alamance/Orange, reflecting our commitment to community care.

Qualifications

• One year of nursing experience required.
• RN license valid in assigned state.
• CPR certification required.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop a plan of care.
• Makes referrals to hospice care disciplines as needed.
• Maintains individualized patient care progress records with accuracy.
• Records pain and symptom management changes/outcomes.
• Educates patients, families, and caregivers on care techniques.
• Communicates regularly with the clinical manager and care team.
• Performs on-call responsibilities as assigned.
• Other duties as assigned.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Qualifications

• One year of nursing experience.
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
