Job summary

Amedisys, a leading home health and hospice provider, seeks an RN Hospice Case Manager to assess patients and coordinate care in the Winston/Guilford or Alamance/Orange territory.

You will work with the care team to develop plans of care, make referrals, document progress, and support families while maintaining compliance with center policies.

Qualifications

• One year of nursing experience.
• Current RN license, state-specific where assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments with the care team to plan and implement a care plan.
• Makes referrals to hospice disciplines as needed or documents rationale.
• Maintains individualized patient care progress records per center policies.
• Records pain/symptom changes and outcomes as appropriate.
• Educates patients, families, and caregivers in care techniques.
• Communicates regularly with clinical manager and care team.
• Performs on-call duties for patients and families as assigned.
• Other duties as assigned.

Attractive pay

• $84,000 - $86,000 + mileage
• Territory: Winston/Guilford or Alamance/Orange

• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Qualifications
