Pride Health seeks an experienced Hospice RN Case Manager to provide comprehensive end-of-life care to patients in their homes across Seven Hills, OH. The RN will coordinate care, perform assessments, develop individualized plans, and support families.
Candidates must have hospice case management experience, proficiency with HCHB, a valid Ohio RN license and CPR, and be able to travel 50–75% while maintaining high-quality, compassionate nursing care.
Seeking an experienced and compassionate Hospice Registered Nurse (RN) Case Manager to provide comprehensive end-of-life care to patients in their homes throughout the Seven Hills, Ohio service area. The RN Case Manager is responsible for coordinating and managing all aspects of patient care, conducting comprehensive assessments, developing individualized care plans, collaborating with the interdisciplinary team, and providing clinical support and education to patients and families. This role requires strong hospice case management experience, proficiency with Homecare Homebase (HCHB), and the ability to manage an independent field-based caseload.
