Pride Global is seeking an experienced Hospice RN Case Manager to coordinate patient care, complete admissions, and work with the interdisciplinary team to deliver high-quality end-of-life care. The role requires strong assessment and communication skills and travel across the Shelby, Fayette, and Tipton counties area.
This position follows a Monday–Friday schedule (8:00 AM–4:30 PM) with mileage reimbursement.
Seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our hospice care team. This role is responsible for coordinating and managing patient care, completing admissions, collaborating with the interdisciplinary team, and ensuring patients receive high-quality end-of-life care in accordance with hospice standards and regulations. The ideal candidate will be flexible, team-oriented, and passionate about providing exceptional patient-centered care.
