Job summary

Amedisys, Inc. is seeking a Part Time Evening Shift Registered Nurse for the Boston area to provide compassionate home health care. The role runs 4:00pm–9:00pm Monday through Thursday, offering flexible hours and a focus on patient comfort at home.

The RN will assess patients, develop and update individualized care plans, coordinate with the care team, and educate families. Valid RN license, CPR, and driver's license are required.

Qualifications

• Current RN license in the applicable state(s).
• One year of nursing experience preferred; new graduate RN candidates may be considered for a program.
• Current CPR certification is required.
• Valid driver\'s license, reliable transportation, and liability insurance.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.
