Job summary

Amedisys, a leading home health and hospice provider, is seeking a Registered Nurse to join its team in Louisiana. The role involves conducting initial assessments, coordinating care plans, and communicating patient progress to the care team to ensure high-quality home-based care.

Qualifications

• Current RN license in the state(s) you are assigned to work.
• One year of nursing experience preferred. If less than one year, RN new graduate program possible.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Maintains continuity of patient care by updating the plan of care with staff.
• Communicates patient progress to the clinical manager, physician, and care team.
• Maintains individualized patient care progress records accurately and timely.
• Educates families on patient condition and needs.
• Performs on-call responsibilities and other duties as assigned.

Responsibilities

• Performs initial nursing assessment on new admissions.


• Completes , coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.


• Regularly communicates patient progress to the clinical manager, physician, and care team.


• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.


• Records pain/symptom management changes/outcomes as appropriate.


• Educates families on patient condition, needs and care.


• Makes referrals to other hospice care disciplines as needed , or documents rationale for not doing so.


• Performs on-call responsibilities and on-call services to patients/families as assigned.


• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.


• One year of nursing experience preferred. If less than one year of nursing experience, you can be considered for the RN new graduate program.


• Current CPR certification.


• Valid driver's license, reliable transportation, and liability insurance.
