Qualifications

  • Minimum 3 years of homecare experience required.

Job Details

We are looking to hire an experienced RN with BSN degree and active New York State RN License in good standing who comes with at least 3 years homecare experience to serve in an RN Team Leader role. This full-time position will function with leadership administrative duties, oversee a team of homecare nurses, precepting new nurses, supervising your peers on their field visits, conducting regular supervisory home visits with nursing staff to ensure clinical competence, to provide guidance in caring for patients with various diagnoses, to address patient/family concerns.

  • In this role, you will also carry a very light patient caseload, and you will coordinate, implement and continually evaluate the home care needs of your patients. You will also review patient insurance information; create and implement a plan of care.
  • Assess the patient for the appropriate level of care and utilization of agency resources.
  • Perform a comprehensive physical, psychosocial and safety assessment via examination, observation and
  • motivational interviewing.
  • Verify the initial plan of care with the patient’s physician; utilizing patient specific parameters inform the physician of any changes in the patients’ status; update the plan of care to reflect changes made by the
  • physician.
  • Ensure the EMR accurately reflects the current plan of care; the services provided; patient’s response to
  • treatment; status of goal achievement.
  • Accurately complete all OASIS data sets, within agency established guidelines, which reflect positive
  • clinical outcomes and consistent utilization of industry established best practices.
  • Perform a comprehensive medication reconciliation the includes all physician prescribed and OTC medications and/or supplements.
