Position Details:

  1. Develops and documents individualized care plans with specific interventions and measurable patient outcomes.
  2. Evaluates progress toward goals and revises care plans as needed.
  3. Establishes, documents, and implements personalized teaching plans for patients and caregivers to promote independence at home.
  4. Performs nursing procedures consistent with homecare protocols, utilizing standard supplies and equipment.
  5. Initiates and participates in regular case conferences, involving all disciplines, including the patient and family, for mutual goal setting and care plan adjustments.
  6. Maintains clinical records, collects data, and prepares reports on activities.
  7. Communicates regularly with the patient's physician, healthcare team, and other key members.

Job Qualifications

Education: Vocational/Technical Diploma in Nursing or Associate's Degree in Nursing required; Bachelor’s degree in Nursing (BSN) preferred.

Experience: 1-2 years RN experience (ICU, CCU, Med-Surg, Oncology, Gerontology) within the last 5 years required, or 1-2 years home health nursing experience or successful completion of LPN to RN competency assessment preferred.

License / Certification: RN license valid in the District of Columbia required; CPR certification required; Valid State Driver's License required upon hire.

This position offers a salary range of $78,000.
