Qualifications

Must have reliable transportation and driver’s license.

Responsibilities

Complete two-week orientation and training.
Assess patients, develop personalized care plans, and ensure optimal outcomes.
Educate patients and families on conditions and care paths.
Collaborate with VNA clinicians and external providers.
Document changes in patient conditions and update plans.
