Maxim Healthcare Services is hiring for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to work with pediatric and/or adult patients in the home setting located in Brookfield, IL.

Salary: $34 - $44 per hour

Responsibilities:

1. Utilize the nursing process to plan, implement, and evaluate patient/client care and/or services provided to clients receiving contracted services, as required.
2. Initiate the plan of care when directed and request revisions from prescribers as patient needs change.
3. Incorporate therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals.
4. Respond to changes in patient/client condition and report such changes to the appropriate physician or supervisor.
5. Administer medications and treatments as prescribed, monitor, and document their effectiveness.
6. Review the plan of care and protocols, and identify areas for healthcare teaching, care coordination, goal setting, and discharge planning as applicable.

Requirements:

1. Current RN license for the state of practice.
2. Current health certificate as per facility or state regulation.
3. Current PPD or Chest X-Ray.
4. Current BLS certification.
5. One year of prior RN experience preferred.
