Job description

Cleveland Clinic has four teams serving 13 counties in the greater Cleveland and Akron areas, each offering specialized services. In this role, you will support the East 2 Team (e.g., upper Submit and lower Cuyahoga). Your duties will include following up on labs, coordinating with care teams, assessing patient charts, and verifying insurance. Cleveland Clinic Home Care provides a comprehensive orientation tailored to your experience to facilitate a smooth transition into this role.

Work Schedule: Days from 8:00 AM - 5:00 PM, with every fourth weekend and holiday requirements.

Key Responsibilities:

1. Provide direct nursing care.
2. Perform accurate admission assessments and reassessments within specified timeframes.
3. Assess patients for safety risks, abuse, and neglect.
4. Coordinate multidisciplinary care for patients.
5. Manage and document patients' pain effectively.

Minimum Qualifications:

• Graduate from an accredited nursing school.
• Current Ohio RN licensure.
• Basic Life Support (BLS) certification through AHA.
• Valid Ohio Driver's License and proof of automobile insurance coverage.
• At least one year of current RN experience.
• Experience in acute Medical/Surgical or Home Care settings.

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN).
• Familiarity with complex medical treatments.
• Experience in homecare, med/surg, ICU, or ED settings.

Physical Requirements:
